When you are charged with the responsibility of caring for a loved one, there is never enough information to assist you. Below is a few selected links to information that Health & Home Services feels like will be of great value to you in keeping your family healthy and safe at home.

Can I change or discontinue the services my family receives?

Health & Home Services encourages a family to update a care plan whenever necessary.  An updated care plan may mean a change in the type or number of services that we provide.  We are aware of changing care situations and can be as flexible as we need to be for the families we serve. The care situation can quickly change, so we're as flexible as possible with your care plan.

How can I verify a shift was completed?

Health & Home Services uses simple timesheets filled out by each caregiver as well as internet software solutions that verify the actual time spent with a patient.  Our tracking system completes a written record of services that can be provided to the family.

Do I need a physician's authorization for your services?

Depending on the services requested, a physician’s order could be required.  Health & Home Services offers a free RN Assessment that can help to determine needs and to coordinate all the necessary authorizations.

What are the costs of your services?

Health & Home Services provides a Personalized Plan of Care specific to every family we serve.  It will take into consideration the health and budget needs.  Cost of services are determined by several factors, including the hours of care per week and the level of home care services needed.

Health & Home Services bills on an hourly basis.  To discuss the exact rates for the services you may need, please contact our office and we are happy to review your Personalized Plan of Care and all costs associated with the plan.

Do I pay the person who delivers my care direct?

Please do not pay any caregiver direct.  The person responsible for payment will be mailed an invoice from Health & Home Services.   The invoice will itemize the care you have received.
